T. Rowe Price 401a Forms
- ENROLLMENT KIT may be obtained by calling (619) 531-5840. You
must complete the Enrollment Form, Designation of Beneficiary Form,
Release, Hold Harmless and Contribution Election Forms to enroll in
the plan. All forms must be submitted to the Deferred Comp Office, Mail
Stop A-49 for processing. Please include your telephone number and use
black ink.
- Change Form - To make changes to name or address (You may only change your contribution to the 457 plan).
- Designation
of Beneficiary Form To change beneficiary designation. Spousal
consent required if beneficiary is other than spouse.

Hardship Certification
To suspend payroll deduction for a minimum of six months.
- Rollover Contribution Transmittal Form - To rollover a qualified Retirement Plan under section 401(a) of the Internal Revenue Code from another plan.
- Special
Tax Notice Regarding Plan Payments - This is a notice of information
needed before deciding how to receive the plan benefits. This includes consideration of rollovers.
- Distribution
Form - To request for payout.
- Provider
to Provider Account Transfer - To transfer account balance from
Hartford to T. Rowe Price, and vice versa.
- Investment Election
Change/Transfer Request - Call plan provider @ 1-800-922-9945.
- Loan Application
