Hartford 401a Forms:
- Enrollment Kit - may be obtained by calling (619) 531-5840.
You must complete the Participant Record, Beneficiary Designation/Spousal
Consent Form, Release, Hold Harmless and Contribution Election Forms
to enroll in the plan. All forms must be submitted to the Deferred Compensation
Office, Mail Stop A-49 for processing. Please include your telephone
number and use black ink.

- To change beneficiary designation. Spousal consent required
if beneficiary is other than spouse.
- Hardship Certification
- To suspend payroll deduction for a minimum of six months
- Rollover Submission - To rollover a qualified Retirement Plan under section 401(a) of the Internal Revenue Code from another plan.

Tax Notice Regarding Plan Payments - This is a notice of information
needed before deciding how to receive the plan benefits. This includes consideration of rollovers.
